You must read each statement and acknowledge you understand the statements herein. The purpose of the Exchange Visitor Program, as administered by the U.S. Department of State (DOS) and the Department of Homeland Security (DHS), is to promote international educational exchange. Host departments have responsibilities for the selection and monitoring of Exchange Visitors. The Chair of the host department and the Principal Investigator/Visiting Scholar Supervisor must read and agree to statements below and demonstrate that agreement by signing this attestation. By signing, the host department attests that it will comply with DOS and DHS regulations.
Notice of Title 18 of the U.S. Code, Chapter 47, Section 1546
Title 18 of the United States Code, Chapter 47, Section 1546 states:
“Whoever knowingly makes under oath, or as permitted under penalty of perjury under section 1746 of title 28,
United States Code, knowingly subscribes as true, any false statement with respect to a material fact in any
application, affidavit, or other document required by the immigration laws or regulations prescribed there under,
or knowingly presents any such application, affidavit, or other document which contains any such false statement
or which fails to contain any reasonable basis in law or fact – shall be fined under this title or imprisoned not more
than 10 years (in the case of the first or second such offense), or 15 years (in the case of any other offense), or
both.“
Please upload a signed letter of invitation. The letter of invitation should come from an appropriate faculty or staff member that has the authority to initiate invitations to visiting researchers, professors, or students. The letter should be on official University letterhead and should outline the exact details of the individual’s visit including dates of program length, any remuneration to be received by the individual from Wake Forest, any other benefits such as housing that will be provided, and a complete description of the purpose of the visit (such as what research will be covered, what meetings may take place, a general weekly schedule, etc.).
Explanation of various J-1 categories:
1) Student Masters: Exchange visitor who will be enrolled on a full-time basis in a Reynolda Campus Master’s program. (Maximum time: No set time; exchange visitor allowed to remain in U.S. until degree is obtained)
2) Student Doctorate: Exchange visitor who will be enrolled on a full-time basis in a Reynolda Campus Ph.D. program. (Maximum time: No set time; exchange visitor allowed to remain in U.S. until degree is obtained)
3) Research Scholar: Exchange visitor coming to the Reynolda Campus to conduct research, learn new techniques, etc. whose program will last for longer than six months. (Maximum time: Five years)
4) Short-term Scholar: Exchange visitor coming to the Reynolda Campus to conduct research, observe, learn new techniques, etc. whose program will not last longer than six months. (Maximum time: Six months)
Important: Please be aware of the fact that a short-term scholar program cannot be extended beyond six months; if you feel that there is even a slight chance that your program will go beyond six months, please select the “research scholar” category.
Once you hit submit below, the above information will be transmitted to the appropriate campus J program responsible officer(s). The responsible officer(s) will contact the EV directly for further biographical and immigration information in order to process the Form DS-2019 that will enable the visitor to apply for the J-1 visa. Once the DS-2019 is processed, the package will be sent to you via inter-campus mail to be mailed out to the visitor. It is strongly recommended that you use a mail delivery service such as FedEx, UPS, or DHL with tracking to ensure delivery. The letter of invitation you produced should be sent along with this visa package.
